Important information regarding your insurance benefits

PLEASE READ PRIOR TO YOUR PROCEDURE DATE

If you have been scheduled for a “screening” colonoscopy and you are not a Medicare recipient, it is your responsibility to contact your insurance company to determine if a screening colonoscopy is a covered benefit.

Please contact your insurance company to check on pre-certification requirements for your scheduled outpatient procedure. If it is required, please let us know as soon as possible so that we can contact the insurance company. Please let us know if this facility is in your “network”, or covered by your insurance policy and you will be rescheduled at one of the local hospitals.

If your insurance is managed care through Medicaid, Medicare, or an HMO, it is your responsibility to contact your Primary Care Provider (PCP) and initiate the referral process. Failure to obtain the necessary referrals may result in rescheduling your procedure. It may also cause you to be responsible for the entire bill.
